  1. Your Name as you would like it to appear on your chart
  2. Your Date of Birth – e.g. 1st
  3. Your Month of Birth – e.g. January
  4. Your Year of Birth – e.g.2014
  5. Your Day of Birth – e.g. Wednesday
  6. Your Zodiac Star Sign e.g. Capricorn
  7. Your Chinese Year – e.g. Snake
  8. The Element related to Zodiac Star Signs – e.g. Capricorn = Earth
  9. The Elements related to your Chinese Year – e.g. Snake = Water
  10. Which is your predominant, most likely to be looked at Star Sign Zodiac or Chinese Year? – this is very important

You will receive a confirmation e-mail once your payment has been received.

Your Crystal Birth Chart etc. will be e-mailed back to you in a pdf format within 2 – 7 days, please allow up to 3 weeks in case of a surge in requests.

If you do not send Sara all the information required please allow 5 – 10 days up to 6 weeks.

We will always endeavour to keep you informed of any delays.
